Have you noticed cytarabine syndrome outside of 7+3 induction? and when you consider it, are there specific labs you could order? I have a lymphoma patient getting BEAM, he started 100 mg/m2 cytarabine BID on recently and started having soon after. He is not neutropenic. He is getting dexamethasone as premedication before cytarabine. Not sure what else to do? The patient is not neutropenic yet.
It is quite rare to see it with bolus cytarabine but I have had a couple of patients in my career get cytarabine syndrome with HiDAC that made it really challenging to get them through their doses. They had very high fevers, rigors, hypotension. All cultures and viral studies were negative. We gave dex and APAP around the clock without much help. Ultimately just pushed through despite the fevers. They resolved immediately after they completed. I don't know there is much else that can be done. Just very close monitoring throughout.
